Attention Deficit Hyperactivity Disorder (ADHD) Referral
For children (between the ages of 0 - 19 at preschool/school age) in Hertfordshire (via HCT - Step2 Service)

In order to support the referral to ADHD assessment services, the following information is required:

  1. Parent/Carer Questionnaire - To be filled in by the child's parent or carer. The more information you include, the more useful it will be when it comes to making an assessment.

  2. Pre-School/School Questionnaire - To be filled out by the child's teacher or the school's special educational needs coordinator (SENCo).

Once the forms are completed, please return them to the reception team and they will let the referring GP know that everything is ready to make the referral. Please be aware that a referral does not guarantee an ADHD assessment as it is possible that your child may not meet the referral criteria and you will be informed of this outcome if necessary.

There are a number of national and local support groups that you can explore while you are waiting for the referral to be processed. Angels is the local support group based in Hertfordshire.
